Welcome to Eye Consultants!  Thank you for choosing our practice for your eye care needs.

About Your Visit

Please allow approximately 2 hours for your new patient evaluation.  Depending on the reason for your visit, your pupils may be dilated. If so, your vision will be blurry and your eyes will be sensitive for 3-6 hours. We recommend having someone to drive you home.


On the day of your appointment, please bring the following:

  • Current eye glasses (distance & near) and/or contact lenses

  • A list of medications you are taking with dosage

  • Eye medications (place bottles in a zip lock bag)

  • A list of allergies to medications

  • Your insurance card (s) and a photo ID

  • A method of payment to cover co-pays, deductibles, or refraction fees, if applicable.

Insurance Referrals

If you have an HMO plan, you must obtain a referral prior to seeing our doctors.  Please contact your primary care physician’s office and provide them with the date of your appointment, the name of the doctor you will be seeing, and reason for your visit.


Please contact our office 2 days before your appointment date to confirm that your referral has been approved and your visit will be covered.

Cancellations

If you are unable to keep your appointment, we ask that you please contact our office as soon as possible to reschedule.  Please provide at least 24 hour notice to avoid a $30 fee.


In case of inclement weather, you will be contacted directly to reschedule your appointment.

Financial Policy

Financal Policy

We are committed to providing you with the highest level of service and quality care.  If you have medical insurance, we will strive to help you receive your maximum allowable benefits.  Ultimately, however, any and all financial liability rests with the patient.


Our office participates with most major medical insurance plans.  Insurances vary in their coverage and it is the patient’s responsibility to understand his/her medical benefits.  Please bring your insurance card(s) to all visits.

Please note our providers do not accept vision insurance (VSP, Davis Vision, Spectera, etc).  The only exception is Eyemed, when linked to a Tufts medical insurance plan. 

Referrals

If you have a managed care plan, you must obtain a referral from you Primary Care Provider (PCP) prior to seeing our doctors.  If your insurance company rejects our claim for services rendered due to lack of referral, you will be held financially responsible for all charges.

Co-payments, Deductibles & Co-insurance

Patients are expected to pay AT TIME OF SERVICE all amounts known not to be covered by their insurance company.  These amounts include co-payments, co-insurance, and/or deductibles.  We accept cash, checks, and all major credit cards for services.

Self-pay

If you have no insurance or we do not participate with your insurance plan, payment is due at the time of your service.  It is always the patient’s responsibility to know if our office is participating with their plan.

Refractions

A refraction is a vision test is not considered a medical service and therefore it is not a covered service by most insurance companies, including Medicare.  If you receive a prescription for glasses, you will be charged $40.00 which is payable at the time of the visit.

Missed Appointments

Please help us serve you better by keeping scheduled appointments. In the event that you are unable to keep your appointment, please give us 24 hour notice.  There will be a $30.00 fee if you fail to show up for any scheduled appointments or cancel the same day as your appointment.

Outstanding Balances

If your account becomes delinquent and you have not established or met payment options with our billing department, your account will be turned over to an attorney’s office for collection efforts.  You agree to pay any related fees.  Any check payments that do not clear the bank will be subject to a $25.00 returned check fee.
